• Student Registration
     

     

     

    Documents needed for registration:

    · Proof of residency (Reference Student Handbook pages 5 & 6):

    1.  Valid Lease or Rental agreement with parent as a lease holder and signed by both parties and GAS or ELECTRIC bill dated within 60 days with the enrolling parent's name and residence address.  If a bill has not been received at time of enrollment then a letter of service from GAS or ELECTRIC may be used.

    Or

    2.  Current Property Tax Bill or Settlement Statement in parent's name and GAS or ELECTRIC bill dated within 60 days with the enrolling parent's name and residence address. If a bill has not been received at time of enrollment then a letter of service from GAS or ELECTRIC may be used.
     
    Or 
     
    3.   An affidavit of residency (the same proof of residency listed above will be required of the owner/leaseholder for an affidavit)
     
     

    · Immunization certificate (Ga. Form 3231)

    · Ear, eye, dental, and nutrition form (Ga. Form 3300)

    · Student’s birth certificate

    · Student’s social security card (or waiver)
