In order to facilitate your request Henry County School System requires a Background check and Mandated Reporter form be submitted for approval. The Authorization of Release of Criminal Background Information form must be notarized and include your social security number. Mrs. White or Mrs. Ralston will notarize in the school office when available or you may use another notary. Once submitted the form can take 2 to 8 weeks for approval. This process only has to be submitted once every three years.
