• Required Athletic Participation Forms

    Hampton Athletics uses the Dragon Fly Max platform to access, complete, and upload ALL required athletic paperwork. You can access instructions for creating an account and completing forms below

    The DragonFly platform contains all mandatory forms. Although, if you need a copy of the Medical Physical to be completed by a doctor, you may download that below. Once completed by a doctor, it can be scanned and uploaded to the DragonFly platform.
