5th Grade AD INFORMATION

 
Acknowledge your student’s accomplishments with their very own yearbook dedication section. Let your child and everyone else know how proud you are of them. Your student will always remember the special message form those they love.

 

2009-2010 PGES 5th Grade ADS

1/8 page: $40.00 - 1 photo maximum/55 words maximum text

 

FINAL DEADLINE:NO ADS WILL BE ACCEPTED AFTER March 19th!

Make checks out to PGES.

 

PLEASE READ CAREFULLY—keep a copy of this form for your records!

1.      The yearbook staff will choose heading color and best format.

                           2.      PLEASE do NOT cut, trim, fold, glue, or tape your photos, they should be in the original shape.

      3.      Please write student’s name and teacher name on back of photo.

                           4.      Please send payment, text, photos and this signed form in a sealed plastic bag or an envelope.

               5.     Photos must be originals. (We cannot be responsible for lost photos.)
6.     All materials MUST BE TURNED IN AT THE SAME TIME:

7.       Please attach your TYPED text for your ad—no handwritten text.

8.   If MAJOR changes are necessary, we will attempt to contact the ad purchaser.
