Is your family moving to the Pate’s Creek community?  And you have a child or children in grades kindergarten through fifth grade?  Welcome!

If you are not sure of the school your child will be attending, you may contact the Henry County School System transportation department at 770-957-2025 for attendance area information.  For driving directions to the school, you may consult our website or call the school at 770-389-8819.

If you are moving during the school year, we encourage you to come to the school to register your child as soon as possible, even before you move if that is feasible.  We realize that adjustment to a new school during the school year is difficult, but the less your child's school year is disrupted by absence, the better.  Ideally, your child should begin attending Pate’s Creek the next school day after he or she withdraws from the previous school.

Registration Requirements

Age:  Kindergarten students must be 5 years old on or before September 1. First grade students must be 6 years old on or before September 1. A copy of the child's proof of age must be supplied at the time of enrollment.

Immunization:  All students in grades K-12 must provide an immunization certificate (Georgia DHR Form #3231) at the time of enrollment. This certificate may be obtained from the county health department or from a physician upon proof of immunization. A list of required immunizations may be obtained from the Georgia Division of Public Health website.

Ear, Eye, and Dental Examination:  All students in grade K-12 must provide a certificate of ear, eye, and dental examination (Georgia DHR Form #3300). This certificate may also be obtained from the county health department or from a physician.

Social Security Number:  All students entering this school system are asked to provide to the school the student's Social Security number. This number will be used as the child’s student number throughout his or her years in school.  For accuracy, it may be easiest for parents to bring the original social security card and the school will make a copy.  Parents may choose not to provide this information by signing a statement indicating that they are declining to provide the social security number.

Transfer Records:  Students transferring into a Henry County school must provide a withdrawal form from the previous school and official notice from that school to verify grade placement at the time of withdrawal. The name and address of the previous school also need to be provided.

Custody:  If any person other than a natural parent has custody of a child, proof of legal custody or guardianship must be provided before the child may be enrolled in a Henry County public school.

Proof of Residence: 

Proof of residence is required when a student initially enrolls in a Henry County school and whenever a change of residence occurs.  The principal or designee shall accept the following records as proof of residency:

1.     A lease or rental agreement consisting of written evidence that the agreement is valid and current, and two current utility bills or verification letter from two of the following utility companies: gas, electric, water, sanitation or cable company. Rental agreements must identify the parent guardian as the tenant of record fiscally responsible for the residence listed in the agreement.  Parent/Guardians listed as occupants of a stated residence are not considered tenants of the property for the purposes of enrollment. All records must include the name and street address of the parent/guardian and service addresses must be plainly visible in any utility document provided as proof;  OR

2.    A Current residential property tax statement, settlement statement, homestead exemption notice, or property deed, and  two current utility bills or verification letter from either the gas, electric, water, sanitation or cable company. Parent/Guardians not listed in ownership documents willing to provide evidence of their legal relationship to the owner shall be permitted to use those documents as an authorized representative of the titleholder.  All records must include the name and street address of the parent/guardian and service addresses must be plainly visible in any utility document provided as proof; OR

3.      A Henry County Schools Affidavit of Residency.

All such documents must be in the name of the student's parent/legal guardian and must consistently identify that individual with the property address of the residence.

When to Register
Our school office is open from 7:30 a.m. to 3:30 p.m. each school day and from 8:00 a.m. to 3:00 p.m. in the summer.  You may come and register your child at any time during those hours.  You need to bring the required documents indicated above.  If your child is transferring from another Georgia public school, you will need to bring proof of residence and a withdrawal form or report card to verify grade placement.  Many of the other required documents will be obtained from the previous school.

If your child attended a Georgia private school or a school in another state or country, you will need to provide a withdrawal form or report card, proof of residence and all of the other required documents.

If you're moving to the area during the summer

We strongly encourage you to come by the school at your earliest convenience to register your children.  We're open every day (Monday through Friday) from 8:00 a.m. to 3:00 p.m. during the summer as we work to prepare for the new school year.  We would love an opportunity to meet you and your children and to get them registered at Pate's Creek Elementary.

Getting your children registered early will allow us to obtain their records from the previous school, giving us important information needed to make an effective class placement for your children.  It will also ensure that your children's teachers will be ready to welcome them at our open house held the week before school begins.
